CONFUSED PLEASE HELP!!!!!!!!

I think the game needs better directions. Im on the first heart transplant and i dont know how to get it out. In the beginning of the instructions it just tells you to drag it out, and I can get the heart out. I would probably enjoy it if I understood so someone PLEASE HELP ME!!! I do not want $5 wasted !!
